| | | Seminar Description: The U.S. Chemical Safety Board (CSB) is an independent federal agency charged with investigating industrial chemical accidents. CSB investigations look into all aspects of chemical accidents, including physical causes such as equipment failure as well as inadequacies in safety management systems, regulations, and industry standards. The CSB investigative staff includes chemical and mechanical engineers, industrial safety experts, and other specialists with experience in the private and public sectors.
A Chemical Safety Board Member has been invited to discuss investigative findings from some of the most recent accidents. The Board currently has 15 investigations open throughout the United States, and over half of these involve accidents at oil refineries. Some of the most recent accidents under investigation include the Silver Eagle Refinery explosion in Utah, Caribbean Petroleum Refining tank explosion in Puerto Rico, Tesoro Refinery Hydrocarbon fire in Utah, and the CITGO Refinery Hydrofluoric Acid release and fire in Corpus Christi. |
